Abstract
Use of particular substituted heterocycle fused gamma-carboline compounds as pharmaceuticals and pharmaceutical compositions comprising them for the treatment of one or more disorders involving the 5-HT2A, SERT and/or dopamine D2 pathways are disclosed. In addition, the compounds may be combined with other therapeutic agents for the treatment of one or more sleep disorders, depression, psychosis, dyskinesias, and/or Parkinson's disease or any combinations.

40 . A method for the treatment of one or more 5-HT 2A -related disorders, comprising administering to a patient in need thereof a Compound of Formula I:
wherein X is —N(CH 3 ), and Y is —C(O)—, in free or pharmaceutically acceptable salt form, in a dose which selectively blocks the 5-HT 2A receptor, wherein the one or more disorders is depression.
41 . The method according to claim 40 , wherein the Compound of Formula I is in pharmaceutically acceptable salt form.
42 . The method according to claim 41 , wherein the pharmaceutically acceptable salt form is a toluenesulfonic acid salt form.
43 . The method according to claim 42 , wherein the dose of the Compound of Formula I is from 2.5 mg to 50 mg, the weight being calculated as the free base form.
44 . The method according to claim 43 , wherein the Compound of Formula I in toluenesulfonic acid salt form is in crystal form.
45 . The method according to claim 43 , wherein the Compound of Formula I in toluenesulfonic acid salt form is in amorphous form.
46 . The method according to claim 40 , wherein the method further comprises administration to the patient of one or more therapeutic agents selected from the group consisting of a 5-HT 2A receptor agonist, a 5-HT 2A receptor antagonist, an ion channel modulator, a serotonin-2 receptor antagonist/reuptake inhibitor (SARI), an atypical antipsychotic, and an antidepressant.
47 . The method according to claim 40 , wherein the method further comprises administration to the patient of one or more therapeutic agents selected from the group consisting of ketanserin, eplivanserin, volinanserin, pruvanserin, ritanserin, nefazodone, serzone, trazodone, amitriptyline, amoxapine, bupropion, citalopram, clomipramine, desipramine, doxepin, duloxetine, escitalopram, fluoxetine, fluvoxamine, imipramine, isocarboxazid, maprotiline, mirtazapine, nefazodone, nortriptyline, paroxetine, phenelzine sulfate, protriptyline, sertraline, tranylcypromine, trazodone, trimipramine, venlafaxine, clozapine, aripiprazole, olanzapine, quetiapine, risperidone, ziprasidone, paliperidone, asenapine, lurasidone, iloperidone, cariprazine, amisulpride, zotepine, and sertindole.
48 . The method according to claim 40 , wherein the Compound of Formula I is used as a sole therapeutic agent.

The method according to claim 43 , wherein the method further comprises administration to the patient of one or more therapeutic agents selected from the group consisting of a 5-HT 2A receptor agonist, a 5-HT 2A receptor antagonist, an ion channel modulator, a serotonin-2 receptor antagonist/reuptake inhibitor (SARI), an atypical antipsychotic, and an antidepressant.

The method according to claim 43 , wherein the method further comprises administration to the patient of one or more therapeutic agents selected from the group consisting of ketanserin, eplivanserin, volinanserin, pruvanserin, ritanserin, nefazodone, serzone, trazodone, amitriptyline, amoxapine, bupropion, citalopram, clomipramine, desipramine, doxepin, duloxetine, escitalopram, fluoxetine, fluvoxamine, imipramine, isocarboxazid, maprotiline, mirtazapine, nefazodone, nortriptyline, paroxetine, phenelzine sulfate, protriptyline, sertraline, tranylcypromine, trazodone, trimipramine, venlafaxine, clozapine, aripiprazole, olanzapine, quetiapine, risperidone, ziprasidone, paliperidone, asenapine, lurasidone, iloperidone, cariprazine, amisulpride, zotepine, and sertindole.
