Abstract

A method may include collecting sensor data related to a set of lifestyle activities related to one or more chronic diseases of a patient. The method may include collecting patient data that indicates at least one of a biological, psychological, or social profile of the patient, and one or more lifestyle choice prescriptions of the patient. The method may include receiving patient input that indicates compliance of the patient with the lifestyle choice prescriptions. The method may include determining a lifestyle choice compliance (LCC) score of the patient based on at least one of the sensor data related to the set of lifestyle activities and the patient input that indicates compliance of the patient with the lifestyle choice prescriptions. The method may include generating an LCC result, wherein the LCC result includes the LCC score of the patient. The method may include providing the LCC result to a care-provider.

What is claimed is: 
     
         1 . A system to evaluate and stratify a lifestyle health compliance of a patient, the system comprising:
 one or more sensors configured to collect sensor data related to a set of lifestyle activities related to one or more chronic diseases of the patient that pertains to at least one of a diet pattern, a sleep pattern, an exercise pattern, an activity level, a heart rate, a posture, a stress, a blood pressure variation, a blood glucose, a heart rhythm, a smoking status, a pain level, and a GPS data;   an electronic health record (EHR) database including patient data that indicates at least one of a biological profile of the patient, a psychological profile of the patient, a social profile of the patient, and one or more lifestyle choice prescriptions of the patient;   a questionnaire module configured to receive patient input that indicates compliance of the patient with the lifestyle choice prescriptions;   a memory configured to store the sensor data, the EHR database, and the lifestyle choice prescriptions; and   a processor, coupled to the one or more sensors, the questionnaire module, and the memory, wherein the processor is configured to perform executable operations including:
 determining a lifestyle choice compliance (LCC) score of the patient based on at least one of the sensor data related to the set of lifestyle activities and the patient input that indicates compliance of the patient with the lifestyle choice prescriptions; 
 generating an LCC result, wherein the LCC result includes the LCC score of the patient; and 
 providing an LCC result to a care-provider. 
   
     
     
         2 . The system of  claim 1 , wherein the lifestyle choice prescriptions include an associated score for full compliance of the patient to avert the one or more chronic diseases. 
     
     
         3 . The system of  claim 1 , wherein the one or more sensors are configured to quantify compliance of the patient with the lifestyle choice prescriptions within a range of values between zero and one. 
     
     
         4 . The system of  claim 1 , the executable operations further comprising scaling the patient input within a range of zero to one and the LCC score of the patient is further based on the scaled patient input, wherein a complete lack of compliance of the patient receives a score of zero and full compliance of the patient receives a score of one. 
     
     
         5 . The system of  claim 1 , wherein each lifestyle choice prescription is determined using a Visual Analog Scale (VAS). 
     
     
         6 . The system of  claim 5 , wherein one or more lifestyle choice prescription compliance targets of the patient are given a separate weighted score between zero and one based on an assigned importance of at least one of the chronic disease and the lifestyle choice prescription. 
     
     
         7 . The system of  claim 1 , the executable operations further comprising:
 collecting LCC scores for one or more similar patients; and   comparing the LCC scores for the one or more similar patients to the LCC score of the patient and the LCC result also include a statistical rank of the patient compared to the one or more similar patients.   
     
     
         8 . The system of  claim 1 , wherein the LCC score has variable weights for each lifestyle choice prescription to accommodate a lifestyle choice prescription importance for a particular chronic disease. 
     
     
         9 . The system of  claim 1 , the executable operations further comprising providing an alert to the care-provider if the LCC score falls below a pre-specified limit determined based on patient input. 
     
     
         10 . The system of  claim 1 , the executable operations further comprising providing an alert to the care-provider if the LCC score falls below a pre-specified limit determined based on the sensor data. 
     
     
         11 . A method to evaluate and stratify a lifestyle health compliance of a patient, the method comprising:
 collecting sensor data related to a set of lifestyle activities related to one or more chronic diseases of the patient that pertains to at least one of a diet pattern, a sleep pattern, an exercise pattern, an activity level, a heart rate, a posture, a stress, a blood pressure variation, a blood glucose, a heart rhythm, a smoking status, a pain level, and a GPS data;   collecting patient data that indicates at least one of a biological profile of the patient, a psychological profile of the patient, a social profile of the patient, and one or more lifestyle choice prescriptions of the patient;   receiving patient input that indicates compliance of the patient with the lifestyle choice prescriptions;   determining a lifestyle choice compliance (LCC) score of the patient based on at least one of the sensor data related to the set of lifestyle activities and the patient input that indicates compliance of the patient with the lifestyle choice prescriptions;   generating an LCC result, wherein the LCC result includes the LCC score of the patient; and   providing the LCC result to a care-provider.   
     
     
         12 . The method of  claim 11 , wherein the lifestyle choice prescriptions include an associated score for full compliance of the patient to avert the one or more chronic diseases. 
     
     
         13 . The method of  claim 11 , wherein collecting sensor data related to a set of lifestyle activities related to one or more chronic diseases of the patient comprises quantifying compliance of the patient with the lifestyle choice prescriptions within a range of values between zero and one. 
     
     
         14 . The method of  claim 11 , the method further comprising scaling the patient input within a range of zero to one and the LCC score of the patient is further based on the scaled patient input, wherein a complete lack of compliance of the patient receives a score of zero and full compliance of the patient receives a score of one. 
     
     
         15 . The method of  claim 11 , wherein each lifestyle choice prescription is determined using a VAS. 
     
     
         16 . The method of  claim 15 , wherein one or more lifestyle choice prescription compliance targets of the patient are given a separate weighted score between zero and one based on an assigned importance of at least one of the chronic disease and the lifestyle choice prescription. 
     
     
         17 . The method of  claim 11 , the method further comprising:
 collecting LCC scores for one or more similar patients; and   comparing the LCC scores for the one or more similar patients to the LCC score of the patient and the LCC result also include a statistical rank of the patient compared to the one or more similar patients.   
     
     
         18 . The method of  claim 11 , wherein the LCC score has variable weights for each lifestyle choice prescription to accommodate a lifestyle choice prescription importance for a particular chronic disease. 
     
     
         19 . The method of  claim 11 , the method further comprising providing an alert to the care-provider if the LCC score falls below a pre-specified limit determined based on patient input.
