Abstract

A medical questionnaire may be provided to a patient and responses to the medical questionnaire may be scored using a scoring procedure associated with the medical questionnaire. The scored responses may then be used to determine a wellness score for the patient. At times, the medical questionnaire may be treatment, diagnosis, and/or symptom specific and the wellness scores determined from the responses thereto may be used to determine a wellness score regarding the patient's wellness with regard to the treatment, diagnosis, and/or symptom. The wellness score may be provided to the patient, a treatment provider, a treatment facility, and/or a treatment administrator. At times a first and second wellness score may be determined and a difference therebetween may be used to determine an improvement score.

I claim: 
     
         1 . A computer-implemented method comprising:
 accessing a pretreatment score database to retrieve a pretreatment score for a medical condition of a patient, the pretreatment score assessing one or more aspects of the medical condition of the patient prior to the patient being treated for the medical condition;   facilitating provision of a medical questionnaire to the patient following the patient being treated for the medical condition by the treatment provided by a treatment provider, the medical questionnaire being associated with one or more scoring procedures stored in a scoring-procedures database and indexed to the medical questionnaire;   receiving responses to the medical questionnaire from the patient, the responses indicating a state of the medical condition following the treatment;   accessing the scoring-procedures database to retrieve the one or more scoring procedures indexed to the medical questionnaire;   evaluating the received responses using the retrieved one or more scoring procedures;   determining a post-treatment score for the medical condition based on the evaluation;   determining an improvement score for the patient's medical condition using the pretreatment score and the post-treatment score;   determining an effectiveness indicator for the treatment provider based on the improvement score, the effectiveness indicator indicating a degree of effectiveness for the treatment provider at providing the treatment; and   storing the effectiveness indicator in a treatment-provider database, the storing including indexing the effectiveness indicator to the treatment and the treatment provider.   
     
     
         2 . The computer-implemented method of  claim 1 , wherein an effectiveness indicator for the treatment provider is determined for a plurality of treatments performed by the treatment provider, thereby creating a plurality of effectiveness indicators, each of the effectiveness indicators being stored in the treatment-provider database and indexed to the treatment provider, the method further comprising:
 accessing the treatment-provider database to retrieve the effectiveness indicators for the treatment provider;   determining an effectiveness score for the treatment provider based on an analysis of the retrieved improvement scores;   storing the effectiveness score in the treatment-provider database.   
     
     
         3 . The computer-implemented method of  claim 1 , wherein for each treatment provider of a plurality of treatment providers:
 an effectiveness indicator for the treatment provider is determined for a plurality of treatments performed by the treatment provider, thereby creating a plurality of effectiveness indicators for each treatment provider, each of the effectiveness indicators being stored in the treatment-provider database and indexed to the treatment provider and treatment performed by the treatment provider, the method further comprising:   for each treatment provider of the plurality:
 accessing the treatment-provider database to retrieve the effectiveness indicators for the treatment provider; 
 determining an effectiveness score for the treatment provider based on an analysis of the retrieved improvement scores; 
 storing the effectiveness score in the treatment-provider database. 
   
     
     
         4 . The computer-implemented method of  claim 3 , further comprising:
 receiving a request for an effectiveness score for at least one treatment provider associated with the treatment from a user;   accessing the treatment-provider database to retrieve the requested effectiveness score; and   facilitating display of the retrieved effectiveness score to the user.   
     
     
         5 . The computer-implemented method of  claim 4 , wherein facilitating display of the effectiveness score comprises:
 facilitating display of the effectiveness score on a geographic map.   
     
     
         6 . The computer-implemented method of  claim 1 , wherein the patient is indexed to an electronic medical record stored in an electronic-medical-record database, the electronic medical record for the patient including information regarding the patient's medical history with regard to the treatment, the method further comprising:
 accessing the patient's electronic medical record, prior to determining the effectiveness indicator for the treatment provider, to retrieve information regarding the patient's medical condition, the determination of the effectiveness indicator being further based on the retrieved information.   
     
     
         7 . The computer-implemented method of  claim 1 , wherein the patient is indexed to an electronic medical record stored in an electronic-medical-record database, the electronic medical record for the patient including information regarding the patient's medical history with regard to the treatment, the method further comprising:
 accessing the electronic medical record of the patient; and   extracting one or more treatment outcome measurements from the accessed electronic medical record, the determination of the effectiveness indicator being further based on the one or more extracted treatment outcome measurements.   
     
     
         8 . The computer-implemented method of  claim 1 , further comprising:
 receiving an indication of a scheduling of performance of the treatment for the patient prior to receipt of the pretreatment score;   determining a pretreatment medical questionnaire to provide to the patient prior to performance of the treatment; and   facilitating provision of the pretreatment medical questionnaire to the patient prior to receipt of the pretreatment score.   
     
     
         9 . The computer-implemented method of  claim 8 , wherein the pretreatment medical questionnaire is associated with one or more pretreatment scoring procedures stored in the scoring-procedures database and indexed to the pretreatment medical questionnaire, the method further comprising:
 receiving responses to the pretreatment medical questionnaire from the patient, the responses to the pretreatment medical questionnaire indicating a state of the medical condition prior to the treatment;   accessing the scoring-procedures database to retrieve the one or more to the pretreatment scoring procedures indexed to the pretreatment medical questionnaire;   evaluating the received responses using the retrieved one or more scoring procedures; and   determining a pretreatment score for the medical condition based on the evaluation.   
     
     
         10 . The computer-implemented method of  claim 1 , further comprising:
 receiving information about at least one of the patient's gender, age, weight, comorbidities, medical condition and state of health prior to the provision of the medical questionnaire to the patient, wherein facilitating provision of the medical questionnaire comprises:
 accessing a database storing a plurality of medical questionnaires, each of the medical questionnaires stored in the database being indexed to one or more treatments; and 
 selecting the medical questionnaire from the plurality of medical questionnaires using the received information and the treatment. 
   
     
     
         11 . The method of  claim 1 , further comprising:
 receiving information about at least one of the patient's gender, age, weight, comorbidities, medical condition, and state of health, wherein the determination of the effectiveness indicator for the treatment provider is further based on the received information.   
     
     
         12 . The computer-implemented method of  claim 1 , further comprising:
 generating a notification for a healthcare provider responsively to a determination that the improvement score is below a minimum threshold; and   facilitating provision of the notification to the treatment provider.   
     
     
         13 . The computer-implemented method of  claim 12 , wherein the notification includes contact information for the patient. 
     
     
         14 . The computer-implemented method of  claim 1 , wherein storing the effectiveness indicator further comprises:
 indexing the effectiveness indicator with at least one of the pretreatment score, the post-treatment score, the improvement score, the treatment, and the patient.   
     
     
         15 . The computer-implemented method of  claim 1 , wherein the facilitating provision of the medical questionnaire to the patient includes providing the medical questionnaire to a personal electronic device operated by the patient. 
     
     
         16 . The computer-implemented method of  claim 1 , further comprising:
 determining a patient compliance question relating to the patient's compliance with post-treatment instructions for recovering from the treatment;   facilitating provision of the patient compliance question to the patient; and   receiving a response to the patient compliance question, wherein the determination of the effectiveness indicator for the treatment provider is further based on the received response to the patient compliance question.   
     
     
         17 . The computer-implemented method of  claim 1 , further comprising:
 determining a patient compliance question relating to the patient's compliance with pretreatment instructions for recovering from the treatment;   facilitating provision of the patient compliance question to the patient; and   receiving a response to the patient compliance question, wherein the determination of the effectiveness indicator for the treatment provider is further based on the received response to the patient compliance question.   
     
     
         18 . The computer-implemented method of  claim 1 , wherein the medical questionnaire is associated with a first scoring procedure and a second scoring procedure, the first scoring procedure being different from the second scoring procedure, the first and second scoring procedures being stored in the scoring-procedures database, the method further comprising:
 separately evaluating the received responses according to the first and second scoring procedures;   determining a first post-treatment score for the medical condition based on the evaluation using to the first scoring procedure;   determining a second post-treatment score for the medical condition based on the evaluation using to the second scoring procedure;   determining a first improvement sub-score for the patient's medical condition using the pretreatment score and the first post-treatment score;   determining a second improvement sub-score for the patient's medical condition using the pretreatment score and the second post-treatment score, wherein the determination of the effectiveness indicator for the treatment provider is based on at least one of the first improvement sub-score and the second improvement sub-score, the effectiveness indicator indicating a degree of effectiveness at providing the treatment for the treatment provider; and   storing the effectiveness indicator in a treatment-provider database.   
     
     
         19 . The computer-implemented method of  claim 1 , wherein a plurality of the treatments are performed by a plurality of treatment providers, each of the plurality of treatments being associated with an effectiveness indicator and each of the treatment providers being associated with a treatment facility, the method further comprising:
 retrieving the effectiveness indicators for the treatment for each of the treatment providers associated with the treatment facility;   determining an effectiveness score for the treatment facility based on the retrieved effectiveness indicators; and   storing the effectiveness score in a treatment-facility database, the storing including indexing the effectiveness score to the treatment facility.   
     
     
         20 . The computer-implemented method of  claim 1 , further comprising:
 providing the effectiveness indicator to a treatment facility administrator.   
     
     
         21 . The computer-implemented method of  claim 1 , wherein the treatment is provided in a treatment facility, the method further comprising:
 selecting received responses relevant to the treatment facility;   evaluating the selected responses using the retrieved one or more scoring procedures; and   determining an effectiveness indicator for the treatment provider using the evaluation of the selected responses.   
     
     
         22 . The computer-implemented method of  claim 1 , wherein the treatment is provided by a first treatment provider and a second treatment provider, the method further comprising:
 selecting a first set of received responses relevant to the first treatment provider;   evaluating the first set of received responses using the retrieved one or more scoring procedures;   determining a post-treatment score for the medical condition for the first treatment provider;   determining an improvement score for the patient's medical condition relevant to the first treatment provider using the pretreatment score and the post-treatment score for the first treatment provider;   determining an effectiveness indicator for the first treatment provider based on the improvement score relevant to the first treatment provider;   storing the effectiveness indicator for the first treatment provider in the treatment-provider database, the storing including indexing the effectiveness indicator to the treatment and the first treatment provider;   selecting a second set of received responses relevant to the second treatment provider;   evaluating the second set of received responses using the retrieved one or more scoring procedures;   determining a post-treatment score for the medical condition for the second treatment provider;   determining an improvement score for the patient's medical condition relevant to the second treatment provider using the pretreatment score and the post-treatment score for the second treatment provider;   determining an effectiveness indicator for the second treatment provider based on the improvement score relevant to the second treatment provider; and   storing the effectiveness indicator for the second treatment provider in the treatment-provider database, the storing including indexing the effectiveness indicator to the treatment and the second treatment provider.   
     
     
         23 . The computer-implemented method of  claim 1 , wherein the patient is associated with an electronic medical record, the method further comprising:
 extracting information from the patient's electronic medical record prior to receipt of the pretreatment score; and   generating the pretreatment score using information retrieved from the patient's electronic medical record.   
     
     
         24 . The computer-implemented method of  claim 1 , wherein the received responses further indicate a patient-perceived effectiveness of the treatment in treating the medical condition. 
     
     
         25 . The computer-implemented method of  claim 1 , further comprising:
 upon an expiration of a predetermined time period measured from performance of the treatment, facilitating provision of a subsequent medical questionnaire to the patient, the subsequent medical questionnaire being associated with one or more scoring procedures and indexed to the medical questionnaire;   receiving responses to the subsequent medical questionnaire from the patient, the responses indicating a state of the medical condition following treatment;   accessing the scoring-procedures database to retrieve the one or more scoring procedures indexed to the subsequent medical questionnaire;   evaluating the received responses using the retrieved one or more scoring procedures;   determining a subsequent post-treatment score for the medical condition based on the evaluation;   determining a subsequent improvement score for the patient's medical condition using at least two of the pretreatment score, the post-treatment score, and the subsequent post-treatment score;   determining a subsequent effectiveness indicator for the treatment provider based on the improvement score, the subsequent effectiveness indicator indicating a degree of effectiveness for the treatment provider at providing the treatment; and   storing the subsequent effectiveness indicator in the treatment-provider database, the storing including indexing the subsequent effectiveness indicator to the treatment provider.
