Abstract

A surgical delivery instrument for delivering a vascular hole closure device having a hole covering member. The delivery instrument comprises a housing, a plunger and an advancer movable within the housing, the advancer having a first portion and a distal portion hingedly connected to the first portion and forming a casing for supporting the hole covering member. Distal movement of the advancer pivots the casing from an angled position to a more linear position to change the orientation of the covering member from a transverse position to a more aligned position. The plunger is advanceable to advance the covering member into the vessel.

         21 . A surgical delivery instrument for delivering a vascular hole closure device having an intravascular component and an extravascular component, the delivery instrument comprising a handle portion, an elongated tube extending distally from the handle portion and an advancer portion positioned distally of the handle portion, wherein the advancer portion includes a first advancer to initially advance the hole closure device distally and the handle portion includes a second advancer to subsequently advance the hole closure device further distally, the advancer portion including a first actuator for moving the first advancer, the second advancer movable independently of the first advancer. 
     
     
         22 . The surgical delivery instrument of  claim 21 , wherein the first advancer advances the hole closure device to change an orientation of the hole closure device within the advancer portion. 
     
     
         23 . The surgical delivery instrument of  claim 22 , wherein distal movement of the first advancer pivots the intravascular component of the vascular hole closure device to change the orientation of the intravascular component from an angled position to a more aligned position. 
     
     
         24 . The surgical delivery instrument of  claim 21 , wherein the first advancer is actuated by a slidable member. 
     
     
         25 . The surgical delivery instrument of  claim 24 , further comprising a stop to limit movement of the slidable member. 
     
     
         26 . The surgical delivery instrument of  claim 24 , further comprising a lockout to lock the slidable member in a distal position. 
     
     
         27 . The surgical delivery instrument of  claim 21 , wherein the advancer portion includes a housing having an angled inner surface engageable by a casing containing the intravascular component to pivot the casing and intravascular component to a more linear position. 
     
     
         28 . The surgical delivery instrument of  claim 21 , wherein the second advancer advances within a lumen of the first advancer. 
     
     
         29 . The surgical delivery instrument of  claim 21 , further comprising a distal stop to limit distal movement of the second advancer. 
     
     
         30 . A surgical delivery instrument for delivering into a vessel a vascular hole closure device having an intravascular component and an extravascular component, the delivery instrument comprising a housing, an advancer movable within the housing, and a plunger, the advancer movable to change a position of the intravascular within the housing and the plunger subsequently movable to advance the intravascular component, and a first actuator for moving the advancer, the first actuator movable independently of the plunger. 
     
     
         31 . The surgical delivery instrument of  claim 30 , further comprising a stop to limit movement of the advancer. 
     
     
         32 . The surgical delivery instrument of  claim 30 , wherein the plunger is movable within the advancer. 
     
     
         33 . The surgical delivery instrument of  claim 30 , wherein the advancer forms a cheater tube having a cut at a distal end to form a hinge and a casing, the casing receiving the intravascular component. 
     
     
         34 . The surgical delivery instrument of  claim 33 , wherein the housing has a curved wall which pivots the casing to pivot the intravascular component to a positioned more aligned with a longitudinal axis of the instrument. 
     
     
         35 . The surgical delivery instrument of  claim 30 , further comprising a slidable member to move the advancer. 
     
     
         36 . The surgical delivery instrument of  claim 30 , further comprising a channel to receive a flexible member connecting the intravascular and extravascular components. 
     
     
         37 . The surgical delivery instrument of  claim 30 , wherein the plunger is in a handle portion of the instrument, and the handle portion locks to the housing of the advancer in a distal position. 
     
     
         38 . The surgical delivery instrument of  claim 30 , wherein the plunger contacts the extravascular component to advance the extravascular component out of the instrument. 
     
     
         39 . The surgical delivery instrument of  claim 30 , wherein retraction of the instrument advances the extravascular component.
