Abstract

Compositions and methods for supporting health, especially renal health, comprising ketonic agents that recapitulate beneficial effects of ketosis by exogenously administered agents. The agents include BHB, analogs thereof, and GPR109A agonists. The agents may further include crystal precipitation inhibitors which synergistically improve treatment of certain renal conditions. The agents may be used in dietary supplements and therapeutic compositions for the treatment of cystic kidney diseases such as polycystic kidney disease, ciliopathies, and other conditions.

What is claimed is: 
     
         1 . A method of supporting renal health in a subject, comprising
 administering to the subject a biologically effective amount of a composition comprising BHB, a BHB analog, or a GPR109A agonist;   wherein supporting renal health is achieved by ameliorating the risk, progression, and/or severity of a renal condition.   
     
     
         2 . The method of  claim 1 ,
 wherein the ketonic composition comprises BHB.   
     
     
         3 . The method of  claim 1 ,
 wherein the ketonic composition comprises an ester of BHB, an analog of BHB comprising a BHB precursor, or 1,3-butanediol.   
     
     
         4 . The method of  claim 1 ,
 wherein the composition is formulated as a dietary supplement.   
     
     
         5 . The method of  claim 1 ,
 wherein the renal condition is acute kidney injury.   
     
     
         6 . The method of  claim 1 ,
 wherein the renal condition is cystic condition of the kidney.   
     
     
         7 . The method of  claim 6 ,
 wherein the cystic condition of the kidney is selected from the group consisting of renal cysts as found in tuberous sclerosis complex, medullary cystic kidney disease, Acquired Cystic Kidney Disease, including acquired simple renal cysts, cystic renal cell carcinoma, and cystic nephroma.   
     
     
         8 . The method of  claim 1 ,
 wherein the renal condition is chronic kidney disease.   
     
     
         9 . The method of  claim 1 ,
 wherein the renal condition is ischemic reperfusion injury of the kidney.   
     
     
         10 . The method of  claim 1 ,
 wherein the renal condition is selected from the group consisting of: diabetic nephropathy, nephronophthisis, Meckel-Gruber syndrome, Bardet-Biedl syndrome, Joubert syndrome, medullary sponge kidney, multicystic dysplastic kidney, Dent's disease, Glomerulocystic kidney disease, Von Hippel-Lindau Syndrome, and mixed epithelial and stromal tumor of the kidney.   
     
     
         11 . The method of  claim 1 , wherein
 the composition further comprises a crystal precipitation inhibitor; and   wherein the crystal precipitation inhibitor is administered in an amount sufficient to inhibit renal crystal formation.   
     
     
         12 . The method of  claim 11 , wherein
 the crystal precipitation inhibitor is a composition selected from the group consisting of citrate, hydroxycitrate, an ester of citric acid, an amide of citric acid, a composition that alkanizes urine, magnesium carbonate, calcium carbonate, sodium carbonate, allopurinol, oxypurinol, febuxostat, an inhibitor of xanthine oxidase, Lesinurad, Rasburicase, and a calcium chelator.   
     
     
         13 . The method of  claim 12 ,
 wherein the crystal precipitation inhibitor comprises citrate.   
     
     
         14 . The method of  claim 11 ,
 wherein the composition comprises BHB.   
     
     
         15 . The method of  claim 13 ,
 wherein the weight percentage of citrate is at least about 20%, calculated as the proportional mass of citrate to the combined mass of BHB and/or BHB analog and citrate.   
     
     
         16 . The method of  claim 12 , wherein
 the renal condition is selected from the group consisting of acute kidney injury, reperfusion injury of the kidney, and chronic kidney disease.   
     
     
         17 . The method of  claim 16 , wherein
 wherein the crystal precipitation inhibitor comprises citrate.   
     
     
         18 . A method of treating a viral infection in a subject in need in treatment therefor, comprising
 administering to the subject a therapeutically effective amount of a ketonic composition, wherein the ketonic composition comprises BHB, a BHB analog, or a GPR109A agonist.   
     
     
         19 . The method of  claim 18 , wherein
 the viral infection is COVID19.   
     
     
         20 . The method of  claim 18 , wherein
 the viral infection is caused by a virus selected from the group consisting of: HIV, a norovirus, a respiratory syncytial virus, a herpes virus, and papillomavirus.   
     
     
         21 . The method of  claim 18 ,
 wherein the ketonic composition comprises BHB.   
     
     
         22 . The method of  claim 18 ,
 wherein the ketonic composition comprises an ester of BHB, an analog of BHB comprising a BHB precursor, or 1,3-butanediol.   
     
     
         23 . The method of  claim 21 , wherein
 the ketonic composition comprises BHB; and   the ketonic composition is co-administered with citrate.
