Claims

Abstract

A suture-based vessel closure device can perform the dilation of an arteriotomy puncture and does not require previous dilation of the arteriotomy puncture by a separate device or by a procedural sheath dilator. The suture-based vessel closure device can place one or more sutures across the vessel access site such that, when the suture ends are tied off after sheath removal, the stitch or stitches provide hemostasis to the access site.

1 . A percutaneous vascular surgical system comprising:
 a sheath having a proximal end and a distal end and defining an inner lumen extending between the proximal and distal ends;   an inflation lumen defined by the sheath;   a reversibly inflatable balloon located about the sheath adjacent the distal end and in fluid communication with the inflation lumen;   a negative pressure generator selectively connectable to the inner lumen; and   a vascular closure device operable through the inner lumen to seal an opening in a wall of a blood vessel through which vascular access of the sheath is achieved.   
     
     
         2 . The surgical system of  claim 1 , wherein a sidewall of the sheath is reinforced. 
     
     
         3 . The surgical system of  claim 1 , wherein the sheath is capable of being pre-shaped. 
     
     
         4 . The surgical system of  claim 1 , wherein the sheath is angulated adjacent the distal end. 
     
     
         5 . The surgical system of  claim 1 , wherein the negative pressure generator comprises a receptacle, a pump, or a syringe. 
     
     
         6 . The surgical system of  claim 1 , further comprising a manually operable valve disposed between the negative pressure generator and the inner lumen. 
     
     
         7 . The surgical system of  claim 1 , further comprising a port adapted for blood pressure monitoring. 
     
     
         8 . The surgical system of  claim 1 , wherein the vascular closure device comprises a closure member movable between a collapsed and expanded state, and a catheter on which the closure member is mounted to allow the closure member to be delivered through the inner lumen. 
     
     
         9 . The surgical system of  claim 8 , wherein the vascular closure device is operable to dispense sealing material through the sheath about the opening in the wall of the blood vessel. 
     
     
         10 . The surgical system of  claim 9 , wherein the catheter comprises an internal bore through which the sealing material may be delivered. 
     
     
         11 . The surgical system of  claim 10 , wherein the sealing material comprises a plug. 
     
     
         12 . The surgical system of  claim 11 , wherein the plug is deliverable along the catheter. 
     
     
         13 . A vascular closure device comprising a closure member displaceable between a collapsed and expanded state, and a catheter on which the closure member is mounted to allow the closure member to be delivered through a lumen in a surgical device to an opening in a blood vessel. 
     
     
         14 . The vascular closure device of  claim 13 , further comprising a sealant delivery system operable to dispense a sealing material proximal of the closure member. 
     
     
         15 . The vascular closure device of  claim 13 , wherein the sealing material comprises a plug deliverable along the catheter proximal of the closure member. 
     
     
         16 . A method of vascular surgery comprising:
 inserting percutaneously a multi-lumen sheath into a blood vessel;   inflating a first balloon located about a distal end of the sheath in order to occlude the blood vessel;   performing a surgical procedure within the blood vessel via the sheath;   generating suction through the sheath during the surgical procedure;   deflating the first balloon;   prior to removing the sheath from the blood vessel, passing a vascular closure device through the sheath;   withdrawing the sheath from the blood vessel; and   sealing an opening in the blood vessel through which the sheath was inserted with the vascular closure device.   
     
     
         17 . A method according to  claim 16 , further comprising:
 passing a closure member of the vascular closure device through the sheath into the blood vessel;   moving the closure member from a collapsed state; and   withdrawing the sheath from the blood vessel with the closure member remaining in the blood vessel.   
     
     
         18 . A method according to  claim 17 , further comprising applying a sealing material about the opening via the sheath. 
     
     
         19 . A method according to  claim 17 , further comprising collapsing the closure member and withdrawing the closure member from the blood vessel via the opening. 
     
     
         20 . A method according to  claim 18 , further comprising applying manual compression against the opening. 
     
     
         21 . A method according to  claim 20 , further comprising generating suction with a vacuum pump to effect a reversal of flow. 
     
     
         22 . A method according to  claim 20 , further comprising applying negative pressure to reverse blood flow in the vessel and surrounding vessels during the vascular intervention. 
     
     
         23 . A method of percutaneous closure of an opening in a blood vessel comprising:
 delivering a collapsed closure member through the opening into the blood vessel;   expanding the closure member;   applying a sealing material about the opening;   collapsing the closure member; and   withdrawing the closure member from the blood vessel through the opening.   
     
     
         24 . A method according to  claim 23 , further comprising delivering the sealing material through a catheter on which the closure member is mounted.
