Abstract
Methods, computer systems, and computer readable media for a patient care card graphical user interface are provided. The patient care card graphical user interface comprises a requirements display area configured to display one or more clinical measures relating to a patient, and a visual indicator that changes depending on whether the at least one or more clinical measures has been met.

One or more non-transitory computer-readable media having computer-executable instructions embodied thereon that, when executed by a computing device, perform a method of determining clinical measures related to a patient and generating representations of the clinical measures in the form of patient care cards, the method comprising:
receiving raw healthcare data associated with the patient from a plurality of raw healthcare data sources; sorting the raw healthcare data into unstructured raw data, structured data of non-standard nomenclature, and structured data of standard nomenclature; transforming at least a portion of the sorted data, wherein:
the unstructured raw data is transformed into structured data of standard nomenclature through natural language processing, and nomenclature and ontology mapping, and
the structured data of non-standard nomenclature is transformed into structured data of standard nomenclature through nomenclature and ontology mapping,
using the structured data of standard nomenclature, determining one or more sets of clinical measures related to the patient using distributed adaptive knowledge processing, the one or more sets of clinical measures comprising contextually-relevant healthcare data specific to the patient; and providing access to a patient care card application, wherein the patient care card application generates a graphical user interface that enables the patient to actively manage the patient's health, wherein the graphical user interface comprises:
a requirements display area that displays a plurality of representations of the one or more sets of clinical measures related to the patient, each representation of the plurality of representations corresponding to a single clinical measure in the one more sets of clinical measures, the each representation of the plurality of representations having a visual indicator indicating whether the patient has met or not met the clinical measure associated with the each representation; and
a measurements display area that displays one or more measurements completed for the patient.
2 . The media of claim 1 , wherein the measurements display area is configured to display a first indicator if the measurement is abnormal, and further wherein the measurements display area is configured to display a second indicator if the measurement is normal.
3 . The media of claim 1 , wherein the measurements display area is displayed simultaneously with the requirements display area and in the same viewable area of the graphical user interface as the requirements display area.
4 . The media of claim 1 , wherein the graphical user interface further comprises a trend display area that displays a trending graph of at least one of the one or more sets of clinical measures related to the patient.
5 . The media of claim 4 , wherein the trend display area is displayed simultaneously with the measurements display area and the requirements display area and in the same viewable area of the graphical user interface as the measurements display area and the requirements display area.

The media of claim 1 , wherein the healthcare data is structured when it is coded using International Classification of Diseases (ICD) codes, and wherein the healthcare data is in standard nomenclature when it comprises one of Systemized Nomenclature of Medicine (SNOMED) or Logical Observation Identifiers Names and Codes (LOINC).
7 . The media of claim 1 , wherein the raw healthcare data is collected by a plurality of data collectors comprising one or more of electronic medical records crawlers, clinical and administrative data collectors, claims data collectors, document feed collectors, and medical device feed collectors.
8 . A computerized method carried out by one or more servers having one or more processors for determining clinical measures related to a patient and generating representations of the clinical measures in the form of patient care cards, the method comprising:
receiving raw healthcare data associated with the patient from a plurality of raw healthcare data sources; sorting the raw healthcare data into unstructured raw data, structured data of non-standard nomenclature, and structured data of standard nomenclature; transforming, using the one or more processors, at least a portion of the sorted data, wherein:
the unstructured raw data is transformed into structured data of standard nomenclature through natural language processing, and nomenclature and ontology mapping, and
the structured data of non-standard nomenclature is transformed into structured data of standard nomenclature through nomenclature and ontology mapping,
using the structured data of standard nomenclature, determining one or more sets of clinical measures related to the patient using distributed adaptive knowledge processing, the one or more sets of clinical measures comprising contextually-relevant healthcare data specific to the patient; and providing access to a patient care card application, wherein the patient care card application generates a graphical user interface that enables the patient to actively manage the patient's health, wherein the graphical user interface comprises:
a requirements display area that displays a plurality of representations of the one or more sets of clinical measures related to the patient, each representation of the plurality of representations corresponding to a single clinical measure in the one more sets of clinical measures, the each representation of the plurality of representations having a visual indicator indicating whether the patient has met or not met the clinical measure associated with the each representation; and
a measurements display area that displays one or more measurements completed for the patient.
9 . The method of claim 8 , wherein the measurements display area is configured to display a first indicator if the measurement is abnormal, and further wherein the measurements display area is configured to display a second indicator if the measurement is normal.
10 . The method of claim 8 , wherein the measurements display area is displayed simultaneously with the requirements display area and in the same viewable area of the graphical user interface as the requirements display area.
11 . The method of claim 8 , wherein the graphical user interface further comprises a trend display area that displays a trending graph of at least one of the one or more sets of clinical measures related to the patient.
12 . The method of claim 11 , wherein the trend display area is displayed simultaneously with the measurements display area and the requirements display area and in the same viewable area of the graphical user interface as the measurements display area and the requirements display area.

15 . A system for determining clinical measures related to a patient and generating representations of the clinical measures in the form of patient care cards, the system comprising:
a computing device having one or more processors and one or more computer-storage media; one or more data stores coupled to the computing device, wherein the computing device:
receives raw healthcare data associated with the patient from a plurality of raw healthcare data sources;
sorts the raw healthcare data into unstructured raw data, structured data of non-standard nomenclature, and structured data of standard nomenclature;
transforms at least a portion of the sorted data, wherein:
the unstructured raw data is transformed into structured data of standard nomenclature through natural language processing, and nomenclature and ontology mapping, and
the structured data of non-standard nomenclature is transformed into structured data of standard nomenclature through nomenclature and ontology mapping,
using the structured data of standard nomenclature, determines one or more sets of clinical measures related to the patient using distributed adaptive knowledge processing, the one or more sets of clinical measures comprising contextually-relevant healthcare data specific to the patient; and
provides access to a patient care card application, wherein the patient care card application generates a graphical user interface that enables the patient to actively manage the patient's health, wherein the graphical user interface comprises:
a requirements display area that displays a plurality of representations of the one or more sets of clinical measures related to the patient, each representation of the plurality of representations corresponding to a single clinical measure in the one more sets of clinical measures, the each representation of the plurality of representations having a visual indicator indicating whether the patient has met or not met the clinical measure associated with the each representation; and
a measurements display area that displays one or more measurements completed for the patient.
