Abstract

Devices and methods are configured to allow transcervical or subclavian access via the common carotid artery to the native aortic valve, and implantation of a prosthetic aortic valve into the heart. The devices and methods also provide means for embolic protection during such an endovascular aortic valve implantation procedure.

1 . A system for aortic valve treatment, comprising:
 an arterial access sheath adapted to be introduced into an access site at the left common carotid artery, right common carotid artery, left subclavian artery, or right subclavian artery, wherein the arterial access sheath includes an internal lumen sized and shaped to receive a valve delivery system configured to deliver a prosthetic valve into the heart through the arterial access sheath;   an occlusion element on the arterial access sheath, the occlusion element adapted to occlude an artery.   
     
     
         2 . A system as in  claim 1 , further comprising a valve delivery system configured to deliver a prosthetic valve into the heart through the arterial access sheath. 
     
     
         3 . A system as in  claim 1 , wherein the occlusion element is a balloon. 
     
     
         4 . A system as in  claim 1 , wherein the sheath has an aspiration port. 
     
     
         5 . A system as in  claim 1 , further comprising a collection reservoir fluidly coupled to the access sheath and adapted to receive fluid from the sheath. 
     
     
         6 . A system as in  claim 1 , further comprising a venous return shunt fluidly coupled to the access sheath and adapted to receive fluid from the sheath for passing to a vein. 
     
     
         7 . A system as in  claim 4 , further comprising an aspiration source coupled to the aspiration port. 
     
     
         8 . A system as in  claim 1 , further comprising a filter coupled to the arterial access sheath to provide embolic protection. 
     
     
         9 . A system as in  claim 8 , wherein the filter is sized and shaped to be deployed across the ostium of the artery. 
     
     
         10 . A system as in  claim 8 , wherein the filter is sized and shaped to be deployed across at least one of the innominate artery, left common carotid artery, or left subclavian artery. 
     
     
         11 . A system as in  claim 8 , wherein the filter is configured to be deployed across the aorta. 
     
     
         12 . A system as in  claim 8 , wherein the filter is integral with the sheath. 
     
     
         13 . A system as in  claim 8 , wherein the filter is removably attached to the sheath. 
     
     
         14 . A system as in  claim 2 , wherein the valve delivery system includes an elongated catheter. 
     
     
         15 . A system as in  claim 14 , wherein the catheter has a length of 60, 70, or 80 cm. 
     
     
         16 . A system for aortic valve treatment, comprising:
 an arterial access sheath adapted to be introduced into an access site at the left common carotid artery, right common carotid artery, left subclavian artery, or right subclavian artery, wherein the arterial access sheath has an internal lumen sized and shaped to receive a valve delivery system configured to deliver a prosthetic valve into the heart through the arterial access sheath;   a filter coupled to the arterial access sheath to provide embolic protection.   
     
     
         17 . A system as in  claim 16 , further comprising a valve delivery system configured to deliver a prosthetic valve into the heart through the arterial access sheath. 
     
     
         18 . A system as in  claim 16 , further comprising an occlusion element on the arterial access sheath, the occlusion element adapted to occlude an artery. 
     
     
         19 . A system as in  claim 18 , wherein the occlusion element is a balloon. 
     
     
         20 . A system as in  claim 16 , wherein the sheath has an aspiration port.
