         21 . A computer-implemented method, comprising:
 receiving a request from a device used by a consumer of medical services to consult with a medical service provider; establishing a communication channel between the device used by the consumer and a device used by a medical service provider engaged with the consumer in an audio-based communication; sending by a computer a message that when rendered by the device used by the medical service provider prompts the medical service provider to furnish information related to the audio-based communication; receiving, by the computer, the furnished information from the device used by the provider; establishing by the computer a work-up record of the audio-based communication with the consumer, with the work-up record comprising consumer entries, recordings, and provider information; and associating, by the computer, the information received from the device used by the provider with the work-up record.

         22 . The method of claim 21 , wherein the information received from the device used by the provider is received after the device used by the consumer disconnects from the communication channel.

         23 . The method of claim 21 , wherein the message further prompts the provider to enter responses to one or more questions using a numeric keypad of a telephonic device; and the method further comprises:
 storing the entered numeric responses with the work-up record of the audio-based communication.

         24 . The method of claim 21 , further comprising:
 retrieving the work-up record and the information associated with the summary of the audio-based communication from a memory device; and sending the work-up record and the retrieved information to the device used by the provider; and sending, to the device used by the provider, information for a graphical user interface to permit the provider to edit one or more of the work-up record and the retrieved information.

         25 . The method of claim 21 , wherein the communication channel comprises one or more of a network-based communication channel, a telephonic communication channel and an Internet-based communication channel.

         26 . The method of claim 21 , wherein the information received from the device used by the provider is medical assessment information that is indicative of the provider's medical assessment of the consumer.

         27 . A computer program product residing on a non-transitory computer readable medium, the computer program product comprising instructions for causing a computer to perform operations comprising:
 receiving a request from a device used by a consumer of medical services to consult with a medical service provider; establishing a communication channel between the device used by the consumer and a device used by a medical service provider engaged with the consumer in an audio-based communication; sending a message that when rendered by the device used by the medical service provider prompts the medical service provider to furnish information related to the audio-based communication; receiving the furnished information from the device used by the provider; establishing a work-up record of the audio-based communication with the consumer, with the work-up record comprising consumer entries, recordings, and provider information; and associating, by the computer, the information received from the device used by the provider with the work-up record.

         28 . The computer program product of claim 27 , wherein the information received from the device used by the provider is received after the device used by the consumer disconnects from the communication channel.

         29 . The computer program product of claim 27 , wherein the message further prompts the provider to enter responses to one or more questions using a numeric keypad of a telephonic device; and the method further comprises:
 storing the entered numeric responses with the work-up record of the audio-based communication.

         30 . The computer program product of claim 27 , wherein the operations further comprise:
 retrieving the work-up record and the information associated with the summary of the audio-based communication from a memory device; and sending the work-up record and the retrieved information to the device used by the provider; and sending, to the device used by the provider, information for a graphical user interface to permit the provider to edit one or more of the work-up record and the retrieved information.

         31 . The computer program product of claim 27 , wherein the communication channel comprises one or more of a network-based communication channel, a telephonic communication channel and an Internet-based communication channel.

         32 . The computer program product of claim 27 , wherein the information received from the device used by the provider is medical assessment information that is indicative of the provider's medical assessment of the consumer.

         33 . An apparatus comprising:
 a processor; and a computer program product residing on a computer readable medium, the computer program product comprising instructions for causing the processor to perform operations comprising:
 receiving a request from a device used by a consumer of medical services to consult with a medical service provider;
 establishing a communication channel between the device used by the consumer and a device used by a medical service provider engaged with the consumer in an audio-based communication;
 sending a message that when rendered by the device used by the medical service provider prompts the medical service provider to furnish information related to the audio-based communication;
 receiving the furnished information from the device used by the provider;
 establishing a work-up record of the audio-based communication with the consumer, with the work-up record comprising consumer entries, recordings, and provider information; and
 associating, by the computer, the information received from the device used by the provider with the work-up record.

         34 . The apparatus of claim 33 , wherein the information received from the device used by the provider is received after the device used by the consumer disconnects from the communication channel.

         35 . The apparatus of claim 33 , wherein the message further prompts the provider to enter responses to one or more questions using a numeric keypad of a telephonic device; and the method further comprises:
 storing the entered numeric responses with the work-up record of the audio-based communication.

         36 . The apparatus of claim 33 , further comprising:
 retrieving the work-up record and the information associated with the summary of the audio-based communication from a memory device; and sending the work-up record and the retrieved information to the device used by the provider; and sending, to the device used by the provider, information for a graphical user interface to permit the provider to edit one or more of the work-up record and the retrieved information.

         37 . The apparatus of claim 33 , wherein the communication channel comprises one or more of a network-based communication channel, a telephonic communication channel and an Internet-based communication channel.

         38 . The apparatus of claim 33 , wherein the information received from the device used by the provider is medical assessment information that is indicative of the provider's medical assessment of the consumer.
