Abstract

A method for providing a user with the ability to access and collect records associated with the user in a secure and private manner includes associating access information with the user for the user to use to access a server storing files comprising records associated with the user in a computer readable storage medium, providing the user with a user interface on a computing device to remotely access the records stored on the server, receiving at the server the files comprising records associated with the user from a service provider associated with the user, receiving at the server a request from the user made through the user interface of the computing device for access to the files, wherein the access information is associated with the request, and sending the user a file containing the records associated with user from the server to the computing device in response to the request.

What is claimed is: 
     
         1 . A method of identifying an individual associated with a health record by using a destination address, the method comprising:
 electronically receiving a health record, the health record directed towards the destination address;   storing the health record in a computer readable storage medium;   determining a user associated with the health record from the destination address, wherein the step of determining the user associated with the health record from the destination address is performed by a computer;   providing access to the health record to the user through use of access information.   
     
     
         2 . The method of  claim 1  wherein the destination address comprises a phone number. 
     
     
         3 . The method of  claim 1  wherein the destination address comprises an email address. 
     
     
         4 . The method of  claim 1  wherein the access information comprises a user name and password. 
     
     
         5 . The method of  claim 1  wherein the access information provides access to a user account of the user. 
     
     
         6 . The method of  claim 1  further comprising receiving consent from the user to send the health record to the destination address. 
     
     
         7 . A method of associating health records with an individual comprising:
 storing a health record in a computer readable storage medium associated with a server;   identifying an individual associated with the health record using a destination address; and   electronically sending the health record to the individual by electronically sending the health record to the destination address.   
     
     
         8 . The method of  claim 7  wherein the destination address comprises a phone number. 
     
     
         9 . The method of  claim 8  wherein the electronically sending is performed using a fax server to fax the health record to the phone number. 
     
     
         10 . The method of  claim 7  wherein the destination address comprises an email address. 
     
     
         11 . The method of  claim 9  wherein the electronically sending is performed by emailing the health record to the email address. 
     
     
         12 . The method of  claim 7  further comprising receiving consent from the individual to electronically send the health the health record to the destination address. 
     
     
         13 . A method for identifying an individual associated with a health record, the method comprising:
 using a computer to individually associate an individual with a destination address, wherein the destination address uniquely identifies the individual;   associating health information with the destination address;   sending the health information to the destination address;   providing a web site associated with the destination address such that the health information sent to the destination address are made available to the individual through the web site, the web site associated with a web server.   
     
     
         14 . The method of  claim 13  wherein the destination address comprises a phone number. 
     
     
         15 . The method of  claim 14  wherein the destination address is a 10 digit phone number. 
     
     
         16 . The method of  claim 13  wherein the destination address comprises an email address. 
     
     
         17 . The method of  claim 13  further comprising receiving permission from the individual to send the health information to the destination.
