System and Method for Facilitating Outcome-Based Health Care

Abstract

In an exemplary embodiment, a server receives from a communication device (i) an identifier of a patient and (ii) a login credential associated with an individual. The server makes a determination based on the login credential that the individual is authorized to access a patient record of the patient. Subsequent to making the determination, the server prepares an assessment based on the patient record, and provides the assessment to the communication device. The assessment includes a measurement. The server subsequently receives a measurement result from the communication device and updates the patient record based on the measurement result. The server prepares a care plan based on the patient record and provides the care plan to the communication device. The care plan includes a task. The server subsequently receives a task status from the communication device and updates the patient record based on the task status.

What is claimed is: 
     
         1 . A method carried out by a server, the method comprising:
 identifying a patient record;   identifying a patient care plan associated with the patient record;   determining an outcome of the patient care plan; and   preparing a subsequent patient care plan responsive to the outcome of the patient care plan.   
     
     
         2 . The method of  claim 1 , wherein identifying the patient record comprises:
 receiving from a communication device (i) a patient identifier associated with the patient record and (ii) a login credential associated with an individual; and   based on the login credential, determining that the individual is authorized to access the patient record.   
     
     
         3 . The method of  claim 2 , further comprising receiving an access permission to allow the individual to access the patient record. 
     
     
         4 . The method of  claim 2 , wherein the individual is chosen from the group consisting of a physician, a nurse, a caregiver, and a family member. 
     
     
         5 . The method of  claim 1 , wherein the patient care plan includes (i) a health condition to be treated, (ii) one or more tasks to treat the health condition, and (iii) a task status of the one or more tasks to treat the health condition. 
     
     
         6 . The method of  claim 1 , wherein determining an outcome of the patient care plan comprises:
 receiving from the communication device patient data indicative of a patient condition;   comparing the patient condition to a previous patient condition stored in the patient record; and   responsive to a difference between the patient condition and the previous patient condition, determining whether the outcome of the patient care plan is a positive outcome.   
     
     
         7 . The method of  claim 6 , wherein the patient data is one of a clinical measurement, blood pressure data, heart rate data, and wound condition data. 
     
     
         8 . The method of  claim 6 , wherein receiving from the communication device the patient data comprises receiving a text message, a voice message, a photo message, a video message, or a HyperText Markup Language file. 
     
     
         9 . The method of  claim 1 , wherein preparing the subsequent patient care plan comprises:
 identifying a patient demographic associated with the patient record; and   determining at least one potential patient care plan that is associated with the patient demographic and a positive outcome.   
     
     
         10 . The method of  claim 9 , wherein the patient demographic is one or more of a patient age, a patient weight, a patient race, and a patient gender. 
     
     
         11 . A system comprising:
 at least one processor;   a non-transitory computer-readable medium; and   program instructions stored on the non-transitory computer-readable medium and executable by the at least one processor to cause the system to:
 identify a care plan associated with a patient; 
 receive patient data indicative of a condition of the patient; 
 determine an outcome of the care plan based on the condition of the patient; 
 prepare a subsequent care plan having one or more tasks to be performed, wherein the subsequent care plan is dependent on the outcome and at least one patient demographic associated with the patient; and 
 provide to the communication device the subsequent care plan and the one or more tasks to be performed. 
   
     
     
         12 . The system of  claim 11 , wherein the communication device is selected from a group consisting of a personal computer, a tablet computer, a telephone, a smartphone, and a feature phone. 
     
     
         13 . The system of  claim 11 , wherein the server is configured to:
 receive from the communication device a task status of the one or more tasks to be performed; and   update a patient record of the patient with the task status of the one or more tasks to be performed.   
     
     
         14 . The system of  claim 11 , wherein the patient demographic is one or more of a patient age, a patient weight, a patient race, and a patient gender. 
     
     
         15 . A non-transitory computer-readable medium having instructions stored thereon, the instructions comprising:
 instructions to receive patient data of a patient;   instructions to update a patient record of the patient with the received patient data;   instructions to prepare a care plan for the patient based on the updated patient record;   instructions to determine an outcome of the care plan, wherein the outcome is a positive outcome or a negative outcome; and   instructions to prepare a subsequent care plan based on the patient record and the outcome of the care plan.   
     
     
         16 . The non-transitory computer-readable medium of  claim 15 , wherein the received patient data is one of blood pressure data, heart rate data, and wound condition data. 
     
     
         17 . The non-transitory computer-readable medium of  claim 15 , wherein the patient record is associated with a patient identifier and a login credential of an individual authorized to access the patient record. 
     
     
         18 . The non-transitory computer-readable medium of  claim 15 , wherein the patient record is associated with a patient demographic, and wherein the patient demographic is one or more of a patient age, a patient weight, a patient race, and a patient gender. 
     
     
         19 . The non-transitory computer-readable medium of  claim 15 , wherein the positive outcome is one of (i) a patient discharge or (ii) a cure of a patient condition. 
     
     
         20 . The non-transitory computer-readable medium of  claim 15 , wherein the patient data comprises a text message, a voice message, a photo message, a video message, or a HyperText Markup Language file.
