Abstract

An acute stroke recanalization system and processes include catheter-based improved reconstrainable or tethered neurological devices which are deliverable through highly constricted and tortuous vessels, crossing the zone associated with subject thrombi/emboli, where deployment impacts, addresses or bridges the embolus, compacting the same into luminal walls which enables perfusion and lysis of the embolus, while the improved neurological medical device itself remains contiguous with the delivery system acting as a filter, basket or stand alone stenting mechanism, depending on the status of the embolus and other therapeutic aspects of the treatment being offered for consideration.

         21 . An acute stroke recanalization apparatus, comprising, in combination:
 a catheter-based apparatus adapted so as to operate in combination with a guidewire effective for accessing of, and emplacement into, the cerebral vasculature, the catheter-based apparatus comprising a guidewire lumen adapted to receive the guidewire, a proximal segment and a distal segment, the proximal segment defining an outer catheter that is configured so as to house the distal segment;   wherein the distal segment comprises an inner catheter and a distal revascularization device, the inner catheter being of suitable length and diameter to be sheathed within the outer catheter, as well as to be capable of being advanced distally out of the outer catheter upon deployment and retracted proximally to within the outer catheter after deployment; and   wherein the distal revascularization device comprises a self expanding radial filter/revascularization and basket assembly that can adopt at least two states per mode, an expanded state, and a retracted state when sheathed within the outer catheter; and   characterised in that, in use, the catheter-based apparatus can be positioned so that the outer catheter is located immediately distal to an embolus located within the cerebral vasculature of a patient, at which point the distal segment is advanced such that the distal revascularization device is deployed and self-expands, and following deployment the distal segment can be withdrawn into the outer catheter causing retraction of the distal revascularization device so as to facilitate removal of the device from the body of the patient.   
     
     
         22 . The acute stroke recanalization apparatus of  claim 21 , wherein the catheter-based apparatus comprises a plurality of apertures allowing infusible lytic agents to exit radially and distally into at least a subject embolus in the cerebral vasculature when transmitted through the guidewire lumen of the apparatus. 
     
     
         23 . The acute stroke recanalization apparatus of  claim 21 , wherein the distal segment comprises struts which permit flexion/extension of the apparatus to navigate through curved vessels. 
     
     
         24 . The acute stroke recanalization apparatus of  claim 21 , wherein the self expanding radial filter/revascularization and basket assembly comprises uniform cell sizes. 
     
     
         25 . The acute stroke recanalization apparatus of  claim 21 , wherein the self expanding radial filter/revascularization and basket assembly comprises variable cell sizes. 
     
     
         26 . The acute stroke recanalization apparatus of  claim 21 , further comprising radiopacity for imaging purposes. 
     
     
         27 . The acute stroke recanalization apparatus of  claim 26 , wherein said radiopacity further comprising at least one marker material selected from the group consisting essentially of platinum and gold. 
     
     
         28 . The acute stroke recanalization apparatus of  claim 27 , wherein the at least one marker material is pressed into pre-laser cut apertures designed to matingly embrace the same. 
     
     
         29 . The acute stroke recanalization apparatus of  claim 21 , wherein the revascularization device is made from at least one of nitinol and spring tempered stainless steel. 
     
     
         30 . The acute stroke recanalization apparatus of  claim 21 , wherein the self expanding radial filter/revascularization and basket assembly is configured such that upon deployment it is able to expand in diameter to the luminal wall of a vessel in which it is located. 
     
     
         31 . The acute stroke recanalization apparatus of  claim 30 , wherein the self expanding radial filter/revascularization and basket assembly is configured such that upon deployment it is able to expand up to a diameter of between at least about 3.5 mm and about 50 mm. 
     
     
         32 . The acute stroke recanalization apparatus of  claim 30 , wherein the radial filter/revascularization and basket assembly is adapted so as to provide a continuous radial pressure when in the expanded state. 
     
     
         33 . The acute stroke recanalization apparatus of  claim 21 , wherein the outer catheter is a stainless steel pusher catheter.
