Transcecal ileostomy set

Abstract

Described herein is a transcecal ileostomy set including a balloon catheter equipped with a blocking balloon, which serves for obstruction of a small intestine, and a fixation balloon, which serves for fixation of the balloon catheter inside the large intestine when inflated. Furthermore, it includes a holder, adapted to be fixed to a patient's body from the outside, which serves for closing a large intestine wall by tightening a bioresorbable loop passing through the large intestine wall and abdominal wall. A rotary part of the plastic holder may be affixed with a protrusion serving as a block when turning to tighten the bioresorbable loop.

1 . A transcecal ileostomy set designed for drainage of intestinal fluids from the small intestine, comprising:
 a balloon catheter adapted to be percutaneously introduced through an abdominal wall and a large intestine wall into the small intestine, including
 a fixation balloon affixed to a distal portion of the balloon catheter, and 
 a blocking balloon affixed to the distal portion of the balloon catheter, 
 wherein a portion of the catheter between the fixation balloon and the blocking balloon is of sufficient length to span the Bauhin valve when the balloons are inflated, 
 whereby, when the fixation balloon is inflated in the large intestine, it fills substantially an entire lumen thereof, and when the blocking balloon is inflated in the small intestine behind the Bauhin valve, it obstructs substantially an entire lumen thereof. 
   
     
     
         2 . The transcecal ileostomy set according to  claim 1 , wherein the balloon catheter comprises three lumens of which a first lumen includes a fixation balloon valve at its proximal end to enable inflation of the fixation balloon, a second lumen includes a blocking balloon valve at its proximal end to enable inflation of the blocking balloon, and a third lumen includes a perforation at its distal end and an intestinal fluid outlet at its proximal end to enable drainage of intestinal fluids. 
     
     
         3 . The transcecal ileostomy set according to  claim 2 , wherein the intestinal fluid outlet is configured to mechanically interact with a bag for the collection of intestinal fluid.

         10 . A method of performing a transcecal ileostomy in a subject in need thereof, comprising:
 providing a transcecal ileostomy set, comprising:
 a balloon catheter adapted to be percutaneously introduced through an abdominal wall and a large intestine wall into the small intestine, including a fixation balloon affixed to a distal portion of the balloon catheter, and a blocking balloon affixed to the distal portion of the balloon catheter, wherein a portion of the catheter between the fixation balloon and the blocking balloon is of sufficient length to span the Bauhin valve when the balloons are inflated, 
 wherein the balloon catheter comprises three lumens of which a first lumen includes a fixation balloon valve at its proximal end to enable inflation of the fixation balloon, a second lumen includes a blocking balloon valve at its proximal end to enable inflation of the blocking balloon, and a third lumen includes a perforation at its distal end and an intestinal fluid outlet at its proximal end to enable drainage of intestinal fluids; 
   inserting a region of the catheter comprising the blocking balloon and the perforation percutaneously into the subject through the abdominal wall and the large intestine and into the small intestine beyond the Bauhin valve;   inflating the fixation balloon in the large intestine, whereby it fills substantially an entire lumen thereof, and   inflating the blocking balloon in the small intestine behind the Bauhin valve whereby it obstructs substantially an entire lumen thereof.   
     
     
         11 . The method of  claim 10 , further comprising inflating the fixation balloon through the fixation balloon valve and inflating the blocking balloon through the blocking balloon valve. 
     
     
         12 . The method of  claim 10 , further comprising connecting a bag adapted for collecting intestinal fluids to the intestinal fluid outlet. 
     
     
         13 . The method of  claim 10 , wherein the transcecal ileostomy set further comprises a holder, wherein the holder comprises a stable part adapted to be fixed to a surface of the human body and a rotary part adapted to tighten a bioresorbable loop by turning, whereby, when the transcecal illeostomy set is in use, the bioresorbable loop purses the large intestine wall around the balloon catheter;
 the method further comprising the steps of:   placing the holder on the subject to hold the portion of the balloon catheter outside the subject,   placing the bioresorbable loop around the balloon catheter in the shape of a purse-string suture to seal a space between the balloon catheter and the incisions edges on the large intestine wall, and   threading both ends of the bioresorbable loop through the abdominal wall such that a lasso-loop is formed.   
     
     
         14 . A method of reversing the transcecal ileostomy of  claim 13  in a subject in need thereof comprising:
 deflating the blocking balloon,   deflating the fixation balloon,   removing the catheter completely from the body of the subject, and   tightening the rotary part of the holder, whereby   the bioresorbable loop purses the large intestinal wall until the wall is completely closed.   
     
     
         15 . The method of  claim 14 , further comprising deflating the balloons with a syringe.
