Abstract

A system for providing feedback to an individual patient for automated remote patient care is presented. A medical device having a sensor for monitoring physiological measures of an individual patient regularly records a set of measures. A database collects the collected measures set, the identified collected device measures set, and the quality of life measures set into a patient care record for the individual patient. A server periodically receives the identified collected device measures set and the quality of life measures set from the medical device, and analyzes the identified collected device measures set, the quality of life measures set, and the collected device measures sets in the patient care record relative to other collected device measures sets stored in the database to determine a patient status indicator. The server further sends automated feedback to the patient based at least in part on the patient status indicator, wherein the automated feedback includes an interpretation of the patient status indicator and related health care information.

         2 . A method for determining patient status and providing feedback to an individual patient for automated remote patient care, comprising:
 organizing patient care records in a database, where each patient care record includes information identifying an individual patient;   for an individual patient, receiving collected device measures to provide raw physiological measures for the patient, wherein the collected device measures are regularly generated and recorded by a medical device comprising a plurality of physiological sensors;   determining derived device measures to provide derivative physiological measures based on the collected device measures;   determining a patient status indicator; and   sending automated feedback to the patient based at least in part on the patient status indicator, wherein the automated feedback includes an interpretation of the patient status indicator and related health care information.   
     
     
         3 . The method according to  claim 2 , wherein the step of sending automated feedback to the patient is selected from a group consisting of sending an e-mail message, sending an automated voicemail message, and sending a facsimile message. 
     
     
         4 . The method according to  claim 2 , wherein the interpretation of patient status feedback is in lay terminology. 
     
     
         5 . The method according to  claim 2 , further comprising sending a notification of potential medical concern to health care professionals. 
     
     
         6 . The method according to  claim 5 , further comprising initiating human contact between the patient and the health care professionals. 
     
     
         7 . The method according to  claim 6 , wherein patient care records include a treatment profile and a medical history. 
     
     
         8 . A system for providing feedback to an individual patient for automated remote patient care, comprising:
 a medical device regularly recording a set of measures, the medical device having a plurality of sensors for monitoring physiological measures of an individual patient, the medical device periodically providing a collected measures set comprising individual measures which each relate to patient information recorded by the medical device;   a database receiving the collected measures sets from the medical device and storing the collected measures sets into a patient care record for the individual patient within a database organized to store one or more patient care records which each comprise a plurality of the collected measures sets;   a server periodically receiving the collected device measures set from the medical device, and analyzing the identified collected device measures set, and one or more of the collected device measures sets in the patient care record for the individual patient relative to one or more other collected device measures sets stored in the database to determine a patient status indicator; and sending automated feedback to the patient based at least in part on the patient status indicator, wherein the automated feedback includes an interpretation of the patient status indicator and related health care information.   
     
     
         9 . The system according to  claim 8 , wherein the automated feedback is provided to the individual patient via e-mail message, automated voicemail message, or facsimile message. 
     
     
         10 . The system according to  claim 8 , wherein the interpretation of patient status feedback is in lay terminology. 
     
     
         11 . The system according to  claim 8 , wherein the server is further configured to send a notification of potential medical concern to health care professionals. 
     
     
         12 . The system according to  claim 8 , wherein patient care records include a treatment profile and a medical history. 
     
     
         13 . A computer-readable storage medium holding code for providing feedback to an individual patient for automated remote patient care, comprising:
 code for organizing patient care records in a database, where each patient care record includes information identifying an individual patient;   code for receiving collected device measures to provide raw physiological measures for the patient, wherein the collected device measures are regularly generated and recorded by a medical device comprising a plurality of physiological sensors;   code for determining derived device measures to provide derivative physiological measures based on the collected device measures;   code for determining a patient status indicator; and   code for sending automated feedback to the patient based at least in part on the patient status indicator, wherein the automated feedback includes an interpretation of the patient status indicator and related health care information.   
     
     
         14 . The computer-readable storage medium according to  claim 13 , wherein the code for sending automated feedback is configured to be sent via e-mail message, automated voicemail message, or facsimile message. 
     
     
         15 . The computer-readable storage medium according to  claim 14 , further comprising code for initiating human contact between the patient and the health care professionals. 
     
     
         16 . The computer-readable storage medium according to  claim 13 , further comprising code for sending a notification of potential medical concern to the patient. 
     
     
         17 . The computer-readable storage medium according to  claim 13 , further comprising code for sending a notification of potential medical concern to health care professionals. 
     
     
         18 . The computer-readable storage medium according to  claim 13 , wherein patient care records of the database include a treatment profile and a medical history.
