Process and system for enhancing medical patient care
Abstract
A process and/or system that extracts selected information from e.g. traditional electronics medical records to enable analysis of a determined medical condition shared by multiple patients. A data warehouse receives the extracted information and reformats that information to enable rapid analysis by a health care provider of that provider's patient population having that medical condition. It further collects the selective data of the selected patients for multiple health care providers and enables comparisons of health care providers' success for such patients to promote upgrading of the treatment by less successful providers.

exact text as granted — not AI-modified1 . A method for monitoring patient care, the method comprising:
receiving a selection of condition or disease; providing an electronic database with patient information, including patient-specific performance parameters for the selected condition or disease, wherein the patient information is automatically extracted from patient electronic medical records (EMR); determining a group of standardized and/or published medical treatment guidelines appropriate for the selected condition or disease; monitoring the patient-specific performance parameters for the selected condition or disease; comparing the patient-specific performance parameters for the selected condition or disease with the determined standardized and/or published medical treatment guidelines for the selected condition or disease; and identifying and displaying patient-specific performance parameters outside the determined standardized and/or published medical treatment guidelines for the condition or disease, where the patient-specific performance parameters are displayed for a plurality of patients identified as having the condition or disease in common, the parameters concurrently displayed for the plurality of patients in an aggregate view.
2 . The method of claim 1 , wherein the patient-specific performance parameters comprise test results.
3 . The method of claim 1 , further comprising triggering an alert when patient-specific performance parameters are outside the standardized and/or published medical treatment guidelines for the condition or disease.
4 . The method of claim 3 , wherein the alert is one of an email message, a telephone message, and an EMR message.
5 . The method of claim 1 , wherein comparing the patient-specific performance parameters for the condition or disease with standardized and/or published medical treatment guidelines for the condition or disease includes identifying candidates for screening and preventative testing.
6 . The method of claim 4 , further comprising providing access to printable educational materials related to the patient-specific performance parameters or the condition or disease.
7 . A system for providing personalized healthcare, the system comprising:
an electronic database automatically populated with patient information, including patient-specific performance parameters for a chronic condition or chronic disease, wherein the patient information is automatically extracted from patient electronic medical records (EMR); a menu comprising a group of user-selectable disease or condition-specific modules, with each disease or condition-specific module including standardized and/or published medical treatment guidelines appropriate for the respective condition or disease specific to each module, the menu configured to receive a user selection of one of the group of modules, where the user selection determines a set of standardized and/or published medical treatment guidelines for the selected module; a healthcare provider display comprising a selected disease or condition-specific module, wherein the disease or condition specific module displays a population of patients with a similar chronic condition or chronic disease corresponding to a selected disease or condition and patient condition-specific data elements for the population of patients based on the automatically-extracted patient information, and where the healthcare provider display further comprises a risk analysis for a condition or disease based on aggregate automatically-extracted patient information including displaying patient-specific performance parameters for the population of patents with the selected disease or condition compared to the standardized and/or published medical treatment parameters for the selected disease or condition; and a user selected patient intervention system for continuous monitoring of patients in the absence of patient-initiated contact, the system automatically producing a communication to a plurality of patients meeting a selection criteria from among the population of patients with the similar chronic condition or chronic disease.
8 . The system of claim 7 , where the selection criteria for the communication include whether patients are overdue for a test or visit.
9 . The system of claim 8 , wherein the disease or condition specific module includes an alert display for patient-specific performance parameters outside the standardized and/or published medical treatment guidelines, and where the communication includes an e-mail.
10 . The system of claim 9 , wherein the alert display identifies at least one of safe medication uses, drug-drug interactions, drug-disease interactions, testing requirements, or undiagnosed potential conditions.
11 . The system of claim 9 , wherein the disease or condition specific module includes user-settable alert triggers.
12 . The system of claim 7 , wherein the disease or condition specific module identifies and displays potential undiagnosed patients for a disease or condition based on automatically-extracted patient information.
13 . A method for monitoring patient care, the method comprising:
receiving a selection of condition or disease; automatically extracting patient information electronically from an existing electronic medical record database, the electronic medical record database containing patient-specific data elements stored in a retrievable manner, where the data is entered in routine patient care documentation by healthcare providers, by staff, or through interfaces with other information systems, and where the patient-specific data elements include patient-specific performance parameters for the selected condition or disease; querying extracted patient information based on criteria, wherein the criteria correlate to the selected condition or disease; identifying a population of patients having the selected condition or disease in common based on the query; determining a group of standardized and/or published medical treatment guidelines appropriate for the selected condition or disease; monitoring the patient-specific performance parameters for the selected condition or disease; comparing the patient-specific performance parameters for the selected condition or disease with the determined standardized and/or published medical treatment guidelines for the selected condition or disease; identifying patient-specific performance parameters for the selected condition or disease outside the determined standardized and/or published medical treatment guidelines for the population of patients; and displaying the identified patient-specific performance parameters for the population of patients, the parameters for the selected condition or disease concurrently displayed for the population of patients in an aggregate view.Join the waitlist — get patent alerts
