Augmentation and repair of tissue defects with in vitro cultured fibroblasts
Abstract
Certain embodiments here in are directed to a method of treating a tissue associated with a defect in a human including wrinkles, rhytids, depressed scar, cutaneous depressions, stretch marks, hyperplasia of the lip, nasolabial fold, melolabial fold, scarring from acne vulgaris, and post-rhinoplasty irregularity. The tissue defect may be treated by introducing a plurality of in vitro cultured autologous fibroblast cells at or proximal to the defect area of the patient's tissue. The autologous fibroblast cells may have been cultured in vitro to expand the number of fibroblast cells in at least one medium that comprises autologous serum. The autologous fibroblast cell cultures may be derived from connective tissue, dermal, fascial fibroblasts, papillary fibroblasts, and/or reticular fibroblasts.

A method of treating a tissue associated with a defect in a human patient using autologous materials derived from the patient, the method comprising: introducing a plurality of autologous fibroblast cells to the tissue at, or proximal to, the defect of the patient after the plurality of autologous fibroblast cells have been cultured in vitro to expand the number of fibroblast cells in at least one medium that comprises autologous serum, wherein the tissue is not a vocal cord of the patient.
2 . The method of claim 1 further comprising introducing collagen to the tissue with the plurality of introduced autologous cells.
3 . The method of claim 1 further comprising introducing autologous serum to the tissue with the plurality of introduced autologous cells.

The method of claim 1 wherein the tissue is associated with a defect chosen from the group consisting of wrinkles, rhytids, depressed scar, cutaneuos depressions, stretch marks, hyperplasia of the lip, prominent nasolabial fold, prominent melolabial fold, scarring from acne vulgaris, and post-rhinoplasty irregularity.
5 . The method of claim 1 wherein extracellular matrix is introduced into the patient in association with the plurality of autologous cells.
6 . The method of claim 1 wherein fibronectin is introduced into the patient in association with the plurality of autologous cells.
7 . The method of claim 1 wherein hyaluronic acid is introduced into the patient in association with the plurality of autologous cells.
8 . The method of claim 1 wherein introducing the plurality of autologous cells further comprises introducing extracellular matrix produced by the autologous cells during culture of the autologous cells.
9 . A method of treating a tissue associated with a defect in a tissue of a human patient using autologous fibroblast cells derived from the patient, the method comprising:
introducing a plurality of autologous fibroblast cells and extracellular matrix to the tissue at, or proximal to, the defect of the patient after the plurality of autologous fibroblast cells have been cultured to produce the extracellular matrix from the fibroblast cells and to expand the number of fibroblast cells in vitro.
10 . The method of claim 9 wherein the fibroblast cells are cultured in autologous serum.
11 . The method of claim 9 wherein the fibroblast cells are cultured in human serum.
12 . The method of claim 9 wherein the fibroblast cells are cultured in serum-free media.
13 . The method of claim 9 wherein the fibroblast cells are cultured in non-human serum.

The method of claim 9 wherein the tissue is associated with a defect chosen from the group consisting of wrinkles, rhytids, depressed scar, cutaneuos depressions, stretch marks, hyperplasia of the lip, prominent nasolabial fold, prominent melolabial fold, scarring from acne vulgaris, and post-rhinoplasty irregularity.
15 . The method of claim 9 further comprising introducing a member of the group consisting of fibrin, fibrinogen, and mixtures thereof to the patient with the fibroblast cells.
16 . The method of claim 9 further comprising introducing extracellular matrix to the patient with the fibroblast cells.
17 . The composition of claim 15 wherein the extracellular matrix comprises collagen or modified collagen.
18 . The method of claim 9 further comprising introducing fibronectin to the patient with the fibroblast cells.
19 . The method of claim 9 further comprising introducing fibrillin or elastin to the patient with the fibroblast cells.
20 . The method of claim 9 further comprising introducing hyaluronic acid to the patient with the fibroblast cells.
21 . The method of claim 9 further comprising introducing a matrix to the patient with the fibroblast cells.
22 . The method of claim 21 wherein the matrix comprises Cellulose, cellulose sponge, gelatin, GELFOAM, bioreactive materials, protein based-based injectables, hydroxyapatite, cordal granules, or porous ceramic implants.
23 . The method of claim 9 wherein the fibroblast cells are expanded from a plurality of cells from a tissue that has been exposed to the sun little or not at all.
24 . The method of claim 9 wherein the fibroblast cells are expanded from a plurality of cells that are enzymatically prepared from a tissue sample.
25 . The method of claim 9 wherein the fibroblast cells are isolated by mechanical treatment to obtain the cells for the introducing into the patient.
26 . The method of claim 9 wherein the fibroblast cells are isolated by enzymatic treatment to obtain the cells for introduction into the patient.
27 . The method of claim 9 wherein in vitro cultured autologous cells are isolated by nonenzymatic treatment to obtain the cells for introduction into the patient.
28 . The method of claim 9 wherein the introduction of the fibroblast cells into the patient comprises a method chosen from injection, engraftment, engraftment by threading, and direct placement.
29 . The method of claim 9 wherein the tissue comprises a member of the group consisting of lower dermis, middle dermis, upper dermis, and skin subcutaneous region.
30 . The method of claim 9 wherein the tissue comprises muscle tissue.
31 . The method of claim 9 wherein the in vitro cultured autologous cells are frozen or thawed prior to placing into the tissue of the patient.
32 . The method of claim 9 wherein the fibroblast cells are connective tissue fibroblasts.
33 . The method of claim 9 wherein the fibroblast cells are fascia fibroblasts.
34 . The method of claim 9 wherein the fibroblast cells are papillary fibroblasts.
