Abstract
According to the invention there is provided the use of melagatran, or a pharmaceutically-acceptable derivative thereof, for the manufacture of a medicament for the treatment of Type I diabetes.

39 . A method of treating Type I diabetes mellitus, which comprises administering a therapeutically-effective amount of melagatran, or a pharmaceutically-acceptable derivative thereof, to a patient in need of such treatment.
40 . A method as claimed in claim 39 , wherein the derivative of melagatran is a prodrug of melagatran.
41 . A method as claimed in claim 39 , wherein the derivative of melagatran is a prodrug of the formula
R 1 O 2 C—CH 2 —(R)Cgl-Aze-Pab-OH,
wherein R 1 represents linear or branched C 1-6 alkyl and the OH group replaces one of the amidino hydrogens in Pab.
42 . A method as claimed in claim 41 , wherein R 1 represents methyl, ethyl, n-propyl, i-propyl or t-butyl.
43 . A method as claimed in claim 41 , wherein R 1 represents ethyl.
44 . A method of improving insulin-independency in patients having Type I diabetes mellitus, which method comprises administering a therapeutically-effective amount of melagatran, or a pharmaceutically-acceptable derivative thereof, to a patient in need of such improvement.
45 . A method as claimed in claim 44 , wherein the derivative of melagatran is a prodrug of melagatran.
46 . A method as claimed in claim 44 , wherein the derivative of melagatran is a prodrug of the formula
R 1 O 2 C—CH 2 —(R)Cgl-Aze-Pab-OH,
wherein R 1 represents linear or branched C 1-6 alkyl and the OH group replaces one of the amidino hydrogens in Pab.
47 . A method as claimed in claim 46 , wherein R 1 represents methyl, ethyl, n-propyl, i-propyl or t-butyl.
48 . A method as claimed in claim 46 , wherein R 1 represents ethyl.
49 . A method of treatment of instant blood-mediated inflammatory reaction, which method comprises administering a therapeutically-effective amount of melagatran, or a pharmaceutically-acceptable derivative thereof, to a patient in need of such treatment.
50 . A kit of parts comprising components:
(a) a first component comprising melagatran or a pharmaceutically-acceptable derivative thereof; and (b) a second component comprising cells, which components (a) and (b) are each provided in a form that is suitable for administration in conjunction with the other.
51 . A kit as claimed in claim 50 , wherein the cells are insulin-producing cells or precursors thereof.
52 . A kit as claimed in claim 50 , wherein the cells are islets of Langerhans.
53 . A kit as claimed in claim 50 , wherein the cells are precursors of insulin-producing cells which are stem cells.
54 . A kit as claimed in any one of claims 50 to 53 , wherein the derivative of melagatran is a prodrug of melagatran.
55 . A kit as claimed in any one of claims 50 to 53 , wherein the derivative of melagatran is a prodrug of the formula
R 1 O 2 C—CH 2 —(R)Cgl-Aze-Pab-OH,
wherein R 1 represents linear or branched C 1-6 alkyl and the OH group replaces one of the amidino hydrogens in Pab.
56 . A kit as claimed in claim 55 , wherein R 1 represents methyl, ethyl, n-propyl, i-propyl or t-butyl.
57 . A kit as claimed in claim 55 , wherein R 1 represents ethyl.
58 . A kit of parts comprising:
(I) one of components (a) and (b) as defined in any one of claims 50 to 57 together with (II) instructions to use that component in conjunction with the other of the two components.
